Initial Comments:
An onsite follow-up survey conducted on September 18, 2023, and September 19, 2023, found that Dialyze Direct PA, LLC had corrected the following deficiencies cited under 42 CFR, Part 494, Subparts A, B, C, and D, Conditions for Coverage of Suppliers of End-Stage Renal Disease (ESRD) Services. The deficiencies were cited as a result of a Medicare recertification survey that was completed on August 10, 2023, resulting in the identification of an Immediate Jeopardy.
As a result of the follow-up survey, the condition level citations cited at: 494.40 Water and Dialysate Quality, 494.100 Care At Home, 494.110 QAPI, and 494.150 Responsibilities of the Medical Director were lifted on September 19, 2023, through verification of implementation of the approved plan of correction.
